For many vulnerable populations, trust is a barrier to care. Our navigators are hired locally and enmeshed in the communities they serve. They are culturally competent and able to build trusted, long-term relationships within the community. This results in an increase in health literacy and connection of patients to key clinical and social services.
Human-Centered Communications
The Docent Health Platform communicates with Medicaid patients over text, phone and email. It can also be customized to a preferred channel or best time of day. Tracking and understanding the non-clinical side of the patient experience is challenging. Often, the last mile in SDoH is a regular collection of social needs as circumstances change. Docent Navigators are enabled with digital tools to collect and manage social risk factors, social needs and social determinants. They are equipped with a resource library of clinical and social services to support patients.
